The venue was not listed. Choices in Boulder include Fiddler's Green Ampthitheater (too cold), the University of Colorado Balch Fieldhouse (too big; maybe unheated); the Boulder Theater, and the Fox Theater. The Fox seems most likely but is not certain. The date is also uncertain. Although the DAT was dated January 14, 1994, the January 14 Santa Cruz Sentinel says that the band was at the Great American Music Hall on "Friday" which, based on the other listings in the paper, meant on the 14th. But the newspaper may have meant the 21st. Whatever the date, this was Moondi Klein's 2nd show as a member of the band (he made a guest appearance the year before) and from the banter it's clearly early in 1994, even if it's not the 14th. The source for this recording was not stated, either. It sounds like a SBD, and the vocals are very clear and distinct, more so than is usual for a stage recording.
